I have used the pain cream and meds as prescribed and have had relief. In fact, I played 18 holes of golf yesterday with no problems. Cream and meds didn't help my swing though ! Now for your bow tie - I LOVE IT ! It brings a certain amount of dignity and class to the event. The wearing of the bow tie may have some personal meaning to you, but once again, I LOVE IT !

I had a vasectomy in October 2008 and experienced various types of pains in my groin area soon thereafter. After about two months, the pain developed into a constant "dull ache" in my left groin area. The pain started around the vasectomy site and radiated outward about four inches. I saw Dr. Parekattil in December 2008 and he started to discuss the option of a Neurolysis for the pain. After his exam, we decided to wait and give it time. Over the next few months, the pain really started to bother me and affect my daily quality of life. It was a constant nagging pain that I could not stop thinking about. The original physician that did the vasectomy decided to go back in and remove the clip on the left side of the vasectomy site. We did this, however, after a few more months the pain was still present. I continued to wait and see if time would alleviate the pain. Unfortunately, by December of 2009 the dull ache was still there.

I scheduled the Neurolysis with Dr. Parekattil in January 2010. The procedure went well and after six weeks, the pain was much better. There is a bit of numbness in the left area just above my groin, but no loss of sensation anywhere else. The procedure did lessen the dull ache. It has been about five months since my Neurolysis and I still feel a bit of an ache sometimes in my left side, but not nearly as bad as before the Neurolysis. I am hopeful that as time goes by, the pain will completely dissipate.
In regards to Dr. Parekattil, he is an incredibly compassionate man. He really cares about this issue and wants to help as much as possible. He is very available and understands that this is a real issue that affects the quality of your life. If you are experiencing "pain" in your groin from a vasectomy (or any other reason) I strongly urge you to see Dr. Parekattil and to consider a Neurolysis.

Who would have known that after 21 yrs of having a vasectomy that I would still be able to conceive a child with my wife. Dr. P was amazing! He explained the whole robotic reversal process and answered any questions and concerns I might have had. The Robotic reversal surgery went through with without a hitch and very minimal pain!!! First of all I would like to start off with a big THANK YOU to Dr. Parekattil. With out his knowledge and skill to pursue this type of cure for pain for so many men, I wouldn't be writing this today. I am a 26 year old man that was diagnosed with testicular cancer in June of 2007. After having an Orchiectomy, I suffered from chronic pain in the area of the testicle that was removed. I went to see several doctors...Urologist, hernia doctors, pain specialist. But unfortunately, no one could help me. I had nerve blocks done. I took pain medication for a couple of years. It got so bad that I became addicted to the pain meds and took myself off of them. Unfortunately, I couldn't function without them. I missed several days of work until I was finally put back on them by my Doctor. At this point I researched on the internet until I came across Dr. Parekattil's name. I sent him an e-mail and he responded to me immediately. Within a day, I spoke to him on the phone and we determined that I was a good candidate for his speciality surgery for this type of pain. I live in California, so we picked a date for the surgery and he had his assistant call me to work out all the details regarding travel and insurance coverage. It all fell into place and within a month I was in Gainesville Florida meeting with Dr. Parekattil. He and his team preformed a robotic abdominal neurolysis and when I woke up in recovery my pain was gone. I couldn't believe it. After close to 3 years of pain it was gone! I don't know how to express what a awesome feeling it is to be pain free. I did have a reoccurrence of some pain which lasted about 5 months. But as of today I am pain free. I owe this to Dr. Parekattil. With his skills and passion to help people like me, the world is a better place. He has a wonderful bedside manner. He took my calls several times prior to the surgery and took his time answering any questions that I had. Also, he was very willing to work with my doctor in California pre op and post op. I was referred to the UF by another doctor who specialized in vasectomy reversals. His schedule was full for six months when I called him. I called the UF and was able to get an appointment for a reversal in about six weeks. I was considering another doctor but, he seemed less personable than Dr. Parekattil. He also seemed to be more of a business man than a Doctor. Dr. Parekattil thoroughly explained the procedure and answered my many questions. I decided to go with UF because they seemed more interested in performing a successful vasectomy reversal than making money. I had a quick visit with Dr. Parekattil the day before the appointment. He met me at his office on a Sunday afternoon and gave me a quick exam. I arrived at the hospital in the morning of the procedure. I filled out paperwork and left for a while. I received a call in the early afternoon that it was time for me to come in. I was taken to a waiting area and rested in a bed while hooked up  to an IV. Eventually I was give some medicine to calm me down before I received the anesthetic. Then I woke up after the procedure was complete. I was sore for the first 4 or 5 days. I did not need to take any pain medication. I simply followed the recovery instructions that Dr. Parekattil gave me. I was able to start running six weeks after the procedure with no discomfort. My wife was pregnant very soon after we were able to have intercourse. Our second daughter was born 11 months after my reverse vasectomy. I had my reversal about 3.5 years after my vasectomy. I was 30 years old at the time of my reversal. I am grateful that my procedure was successful and that everyone at UF took such good care of me. I am still amazed that my wife was pregnant so quickly.

Dr. Parekattil is the best! My neurolysis procedure was 100% successful in eliminating all pain without affecting any other sense of sensation/touch in the area. The recovery was limited to mild discomfort, far less than the vasectomy itself. The people at Shands were great and my insurance applied, which would not have been true with reversal.

The most difficult part of the choice between reversal and neurolysis is that as guys we want to fix things, so a reversal seems like the right approach because it puts the pipes back together. But the data doesn't support it as a solution: 60-70% success rate with reversal vs 90% on the first try with neurolysis.

I am 50 years old and understand testicular pain.  My pain started in the right testicle in April of 2007. Over a few days, the pain increased to become constant and sometimes throbbing.  My only relief was to take Vicodin and lay down.  My quality of life had changed dramatically.  I went from being very active, able to do physical work and exercise to not being able to perform simple tasks due to the pain.  After several courses of different antibiotics that failed to alleviate the pain, I had surgeries in 9/07 (granuloma removal), 11/08 (epididymitis removal) and 2/10 (2 cysts, vas deferens, and adhesions removed). The first two surgeries alleviated the pain for months, but the last surgery alleviated the pain for only two weeks. During my post surgery visit at my Urologists office on 3/4/10, I asked for testicle removal as it was the only surgery left that I thought could relieve my pain. I was living a nightmare and just wanted the pain to stop. At this office visit my Urologist wisely referred me to Dr. Parekattil for a consultation.

I met with Dr. Parekattil and his staff on 3/30/10. He and his staff are very professional and completely explained the denervation procedure.  After the explanation, both my wife and I realized denervation is a very logical approach to pain elimination-cut the nerves and the pain will stop. On 5/3/10, I went into surgery with my right testicle hurting as usual.  When I woke up in recovery I felt something that I hadn’t felt in months, no testicle pain!  The incision site was hurting slightly as expected, but only required additional pain medication for the first 8 hours.  The staff at Shands Hospital was caring, kind and considerate of my situation.

It is now two weeks since my surgery and the incision site pain/bruising is just about gone. I don’t have any numbness and feel very normal.  It is impossible to put into words the depth of gratitude I have for Dr. Parekattil and his staff.  His innovative denervation procedure COMPLETELY eliminated my testicle pain without a more invasive surgery. Dr. Parekattil has given me a new life as my perspective on people and life has changed.  I am happy again about living and want to encourage men who have chronic testicular pain that denervation can be a viable solution.  Without hesitation, I recommend visiting Dr. Parekattil

I had suffered from severe chronic R testicular pain for thirteen (13) years. It was treated primarily with pharmacotherapy and pain clinic administered nerve blocks with a mixture of local anesthetic and methylprednisolone. The most benefit was from periodic therapeutic dosed spermatic cord blocks. When this pain became aggravated and very severe, therapeutic dosed periodic cord blocks no longer gave a prolonged pain reduction and I then attended two (2) of the best pain clinics with one leaning towards surgical cordotomy at the spinal level with no real percentage of success or outcome. The other offered radio frequency ablation (temporary neurotomy) at T-8 to T-10 with the warning that their experience with this procedure is that it usually only last three (3) months and the pain fully returns. Since I had already previously had anesthetic blocks at T-8 to T-10 years ago and had no relief of pain this procedure was not appropriate as successful blocking of pain through these paravertabral blocks is a prerequisite before radio frequency ablation. Other options included continuous local anesthetic/opiate injection at the spinal level controlled by a permanent pump and electronic stimulator implant at the spinal cord (spinal cord stimulation). Neither of these seemed a good option as I would permanently have a catheter in my spinal epidural space moving around with the pump always needing top up and I would be constantly dealing with the side effects of the medicine or in the case of spinal cord stimulation the electrical leads have a tendency of breaking off where they come out of your back.

I met Dr. Sijo Parekattil, a Cleveland Clinic trained microsurgeon specializing in chronic testicular pain now practising at University of Florida/Shands Hospital while searching for a pain clinic on the internet. It was immediately obvious to me that Dr. Parekattil understood exactly what chronic pain actually is as he answered a question on an online health discussion forum for chronic testicular pain describing it as "nerve malfunction that triggers at a very low threshhold.... with no definable abnormality on imaging studies". This is precisely what I have been suffering from as my pain was idiopathic in nature and centered on the top pole of my R testis. Dr. Parekattil's health forum answer continued "we have developed a new option called robotic microsurgical neurolysis of the spermatic cord". I immediately contacted Dr. Parekattil via email which was promptly returned by a phone call with Dr. Parekattil. I found Dr. Parekattil to be a very patient, gentle and determined physician who's knowledge and understanding on chronic testicular pain by far exceeded any other physician that I have spoken with.  Dr. Parekattil had solid positive procedural outcomes having performed many robotic cord neurolysis procedures. His success rate of 86% exceeded any other method for performing this procedure. Most importantly the relief of pain is permanent. An appointment for preoperative consultation and for the cord neurolysis procedure was booked with very little wait time. The most common subinguinal neurolysis procedure is relatively minor. The incision is very small and heals quickly. On operating day I had very little anxiety knowing that Dr. Parekattil was the surgeon. No other surgeon performs the neurolysis procedure quite like Parekattil as he has tweaked on it to obtain the best outcomes. The anesthetist administered an iv general anesthetic before the bed started rolling from the preop hold room so I did not have the anxiety or memory of being rolled into the operating room. I woke up in the postop recovery room and walked out of the hospital a short time later. The subinguinal spermatic cord neurolysis is a rather trivial procedure and in most cases this is all that is required to obtain complete relief of pain in 76% of all subinguinal neurolysis procedures or partial relief of pain, scoring 5 or less on a pain score of 0-10 in 10% of all subinguinal neurolysis procedures, so its not bothersome. In very severe cases of testicular pain he has had success with inguinal cord neurolysis and in extreme cases abdominal cord neurolysis. My R testicular pain is essentially gone. Its is important for patients to understand that this procedure will only relieve testicular pain, the associated pain that travels up the spermatic cord and the pelvic area located just above the scrotum. I had no loss of feeling, just relief of pain.

I would strongly recommend Dr. Parekatill for any person suffering from testicular pain that wants the best possible outcome.   Delaying or trying to ignore the pain for more than 6 months will result in the pain being more difficult to remove.
